New Report Details VHAN’s Impact in 2020

In 2020, VHAN focused on 11 key aims to drive quality performance and improve patient care. Our new Impact Report outlines how the network performed across these key aims, including several personal stories that detail how VHAN providers have made a difference in patients’ lives.

One such story showcases how the VHAN Pharmacy Team worked with a 70-year-old patient who suffered from uncontrolled diabetes, impaired cognition and memory loss. With help from her providers and a VHAN pharmacist, the patient resolved 27 medication discrepancies, resulting in her A1c levels dropping from 13.6% to 7.7%. Her pain and chronic fatigue have improved, and she has managed to avoid unnecessary ER visits.

The inaugural Impact Report provides a snapshot of how VHAN members have leveraged network tools and support to provide vital medication management services, behavioral health support, wellness visits, appropriate cancer screenings and more.
