Round-Up of Resources for National Diabetes Awareness Month

In Tennessee, 13.8%, or more than 710,000 adults, have been diagnosed with diabetes. In most southern states, more than one-third of residents are estimated to have prediabetes. Combined with high poverty levels and lower rates of access to health insurance and healthcare in general in the southern U.S., management of chronic conditions, such as type 2 diabetes, becomes a distinct challenge for patients who live in these regions and their care teams.

We have gathered several resources that health care professionals can use to help their patients manage diabetes.
