VHAN Pharmacy Support Helps Improve Hypertension Control
Since fall 2024, VHAN Care Management nurses and pharmacists have partnered with Vanderbilt Population Health pharmacists through the Heart and Kidney Health Program to improve blood pressure control among patients with decreased kidney function.
The program offers eligible patients education, nurse coaching and pharmacist-led medication management over several months.
Patients who completed the program saw an average improvement of 11 mmHg in systolic blood pressure and 4 mmHg in diastolic blood pressure. In one particularly successful case, a pharmacist worked directly with a primary care provider to help lower a patient’s blood pressure from 156/79 to 133/69.
This collaborative approach is one of the benefits of VHAN membership. The VHAN Pharmacy Team can support practices caring for patients with uncontrolled chronic conditions, including hypertension and diabetes.
