Care Management

Support for VHAN Patients

Vanderbilt Health Affiliated Network (VHAN)’s Care Management program brings together a team of registered nurses, diabetes educators, social workers and pharmacists to help patients achieve their health goals. If you’re a patient recovering from a recent hospitalization, trying to better manage a chronic disease or navigating a complex medical condition, VHAN’s team of care navigators can provide personalized care for your needs. Contact your VHAN physician to get started with this program or watch this video to learn more.
