Vanderbilt Health Affiliated Network (VHAN)’s Care Management program brings together a team of registered nurses, diabetes educators, social workers and pharmacists to help patients achieve their health goals. If you’re a patient recovering from a recent hospitalization, trying to better manage a chronic disease or navigating a complex medical condition, VHAN’s team of care navigators can provide personalized care for your needs.
