The VHAN diabetes team continues to make telehealth more personal, particularly in the area of remote patient monitoring (RPM).

VHAN’s multidisciplinary team of pharmacy experts includes Ashley Sigg, a Transitional Care Management Pharmacist who offers medication reconciliation and support for 30 days after a patient’s hospital discharge.

The new Aetna Medicare Advantage (MA) contract is measured across quality and cost performance, and participation means expanded access to high-value, low-cost care for MA patients across the network. The network’s new agreement with Aetna brings VHAN to three MA contracts, alongside Cigna MA and Humana MA.

NexusACO is now available to self-funded and fully insured employers in Nashville, Knoxville and Chattanooga. VHAN providers will be featured in Tier 1 of the NexusACO network, with members incentivized through lower copays to choose these physicians for their care.

The Tennessee Chamber of Commerce & Industry announced a new collaboration with Aetna and VHAN to provide small businesses access to Aetna Funding Advantage self-funded health plans at a significant discount. The plan, known as TN Chambers Health, will leverage trusted VHAN providers across Tennessee to care for plan members.

For the second year in a row, VHAN achieved 100% performance for all Aetna contracted quality measure targets during the 2019 performance year. VHAN’s performance actually exceeded Aetna’s national standards in several measures.

VHAN has announced a $7.5 million shared savings distribution that will be paid to provider members in late 2019. These incentives were achieved thanks to the diligent work of network providers, who have now driven savings for the fifth consecutive year.

The Cigna Collaborative Care initiative is designed for people covered by a Cigna commercial plan or Cigna-HealthSpring Medicare Advantage plan who receive care from clinicians and facilities participating in VHAN.
