Over the past 11 years, VHAN has created $204 million in shared savings for VHAN health plans, resulting in $77 million in shared savings and quality bonuses delivered to members. This achievement is a direct result of our network members’ shared commitment to providing high-quality, low-cost patient care.

Through our collaborative partnership and focus on data-driven approaches to changing care delivery, we are realizing better outcomes and measurable health improvements across the state and region.

Social work is an integral part of VHAN’s Care Management team, providing a direct connection to patients and caregivers. This was the case with a 64-year-old patient who was diagnosed with multiple sclerosis and several other conditions. After a hospitalization, the patient moved in with her daughter and husband, but she still needed more help to manage her health at home. That’s when VHAN’s social work care navigators stepped in.  

To support VHAN members and patients with behavioral health needs, VHAN offers a pediatric behavioral health consult line. Here’s a real-world example of how the consult line can change lives.

When VHAN patients obtain care outside the network, the opportunity to impact the cost and quality of that work is diminished if not eliminated. A key priority for the network is access, and in 2020 VHAN met or exceeded in-network access and utilization goals.

The Vanderbilt MyHealth Bundles program is an innovative way to attract new patients and has succeeded in expanding the network. Here are two stories of how bundles have improved patient care and enabled cost savings for patients and providers. 

Diabetes and its associated complications such as dialysis and heart disease not only negatively impact patients’ lives, but they are also costly. Here’s a story of how a VHAN diabetes program helped one patient better manage her condition.

Working directly with our member hospital systems and practices, VHAN aims to improve the transition process and improve continuity of care, thus reducing adverse events and readmissions. 

In 2020, the VHAN Pharmacy Team worked with about 1,400 patients to optimize medications for chronic disease management, transitions of care and polypharmacy. Here’s one example of how VHAN pharmacy support helped one patient manage her complex medical conditions.

VHAN has aggregated best practices for preventive screenings into a quick reference guide that focuses on practical ways to improve workflows and patient engagement to increase breast, colorectal and cervical cancer screenings.
