2022 Shared Savings Results From United and Aetna MA 

Over the past 11 years, VHAN has created $204 million in shared savings for VHAN health plans, resulting in $77 million in shared savings and quality bonuses delivered to members. This achievement is a direct result of our network members’ shared commitment to providing high-quality, low-cost patient care.

Through our collaborative partnership and focus on data-driven approaches to changing care delivery, we are realizing better outcomes and measurable health improvements across the state and region.

VHAN Social Work Team Supports Patients and Caregivers

Social work is an integral part of VHAN’s Care Management team, providing a direct connection to patients and caregivers. This was the case with a 64-year-old patient who was diagnosed with multiple sclerosis and several other conditions. After a hospitalization, the patient moved in with her daughter and husband, but she still needed more help to manage her health at home. That’s when VHAN’s social work care navigators stepped in.
